The reporting of adverse incidents is crucial to helping us develop our safe practice guidelines, as well as providing research data and information for our insurers and other interested parties. Any named parties remain confidential and are only shared when required by our brokers, insurers or lawyers.
You should complete the AIR form whenever you believe a patient has experienced an adverse incident, no matter how minor. Sometimes an adverse incident may not be immediately apparent – for example if a nerve has been irritated during treatment – with the extent of the injury only becoming clear after the patient has left the clinic.
